To ensure a higher quality of health care, Elevated Primary Care limits patient enrollment and runs on a membership-based model. For an annual fee, patients are afforded:
- Flexible appointment times that are typically available within days;
- Office visits that are thorough, not rushed;
- Convenience of limited to zero wait times for office visits, in-office lab draws, and EKG readings;
- Physician support that extends to acute care settings – Dr. Kowalski admits patients to Northwest Medical Center, Broward Health Coral Springs, Solaris HealthCare of Coconut Creek, and Encompass Rehabilitation Hospital of Sunrise;
- Better access to a well-established network of trusted specialists to guide you to cover a broad array of medical disciplines.
Membership with Elevated Primary Care is not an insurance plan and is not meant to take the place of your existing insurance coverage.
